For x-ray installations, review the enclosed checklist (PDF) and return to ProPhysics for processing. The work cannot be started until all pertinent information has been received. In general, when all the information is obtained, we issue a report within five business days. Please contact our office to discuss any considerations necessary for completion of the plan review.

For radioactive materials installations, fill out the enclosed form (MS Word) and return to ProPhysics for processing. ProPhysics personnel will contact the facility to obtain additional information regarding the usage of radioactive materials within space in question. In general, when all the information is obtained, we issue a report within five business days.
